This program explores how professionals and community-based organizations can better support older adults and families navigating the challenges of dementia care. Professionals are often called upon during moments of crisis — when families are overwhelmed by behavior changes, safety concerns, caregiver burnout, conflicts about care decisions, or uncertainty about available services and next steps. The session highlights the emotional and practical realities families face and emphasizes the importance of person-centered, compassionate, and informed support. LTCCC’s free Dementia Care in the Community Toolkit is a collection of practical, accessible resources designed to help families and caregivers manage common dementia-related challenges. The program demonstrates how professionals can use these tools to strengthen caregiver education, reinforce care planning conversations, support dementia-related problem-solving, and connect families with trustworthy information and guidance.
