Problem Facilities: SFFs and One-Star Nursing Homes (2021)

Special Focus Facilities (SFFs) and SFF Candidates are nursing homes that have a history of serious quality issues or are included in the Centers for Medicare & Medicaid Services (CMS) program to stimulate improvements in their quality of care. One-star facilities are assigned the lowest overall rating (one out of five stars) based on health inspections, staffing, and quality measures. This page contains facility-level data on each state’s SFFs, SFF Candidates, and one-star facilities. Download individual state files by clicking the state on the map or list below.
