June 16, 2025 – Today LTCCC is releasing its new Dementia Care in the Community Toolkit, a free online resource developed to support high-quality, person-centered dementia care for individuals living at home or in assisted living.
A growing number of people living with dementia receive care outside of nursing homes, yet there are no federal standards for dementia care in these settings. This often leaves individuals, families, and caregivers without clear guidance or protections.
The Dementia Care in the Community Toolkit addresses this critical gap by offering practical, easy-to-use materials grounded in professional standards and real-world care experiences.
This toolkit includes:
- Fact sheets on topics ranging from home safety to psychotropic drugging for family caregivers and direct care staff;
- Corresponding “Dementia Care Cards,” which offer quick, easy-to-use tips drawn from our fact sheets to give families and caregivers practical support for day-to-day care;
- A curated collection of external resources to deepen knowledge and connect users with trusted tools, guides, and programs.
This free resource is designed for family members, care teams, and providers who are committed to ensuring compassionate and appropriate care for people with dementia – wherever they live.
