What is a Double G Citation?

“Double G” cases refer to a CMS policy intended to identify and address facilities with a historical pattern of high-level noncompliance. Double G citations occur when a facility with a previous citation at a G level is cited again at a G level. G level citations are considered “actual harm” or above on the Scope and Severity Grid. Facilities with Double G citations must face specific and automatic penalties.
