November 26, 2024  – Federal law mandates that all nursing homes provide sufficient staff to meet the care and quality of life needs of their residents every day. However, new data show that hundreds of thousands of residents live in facilities that are staffed far below the minimum needed to provide basic clinical care, as identified in a landmark 2001 federal study.

According to LTCCC’s latest nursing home staffing report, based on CMS’s Q2 2024 payroll-based journal data, over 70% of US nursing homes are failing to provide the 4.1 hours per resident day (HPRD) of nursing staff identified as necessary to provide basic care in the 2001 study.

In addition to information on nurse staffing levels, LTCCC’s staffing report also provides detailed data on various non-nurse staffing categories, including the presence of medical directors, therapists, and activities staff, and the extent to which facilities are using contract staff. It also includes summary national, regional, and state findings.

New York Staffing Facts for Q2 2024

Nurse Staffing

  • NY nursing homes reported an average of 3.59 total nurse staff hours per resident day (HPRD)This puts NY in the bottom ten states in respect to average nurse staffing.
  • 80% of NY nursing homes provide less nursing staff time than the minimum necessary to keep residents safe (4.1 HPRD, as determined by the landmark 2001 federal study.)
  • Top five nursing homes: Henry J Carter Skilled Nursing Facility (Manhattan), Queen of Peace Residence (Queens), Woodland Pond at New Paltz (New Paltz), Northwell Health Orzac Center for Rehabilitation (Valley Stream), and Kendal at Ithaca (Ithaca).
  • Bottom five nursing homes: Haven Manor (Queens), Ocean Garden (Queens), Livingston Hills (Livingston), Renaissance (Staatsburg), and Newfane Rehab & Healthcare Center (Newfane).
    • Note: These nursing home rankings exclude outliers, such as pediatric facilities and one reporting no nurse aide time.
  • The use of contract staffing dropped slightly from 12.7% to 12.2%, reflecting a continued decrease in the use of contract staff since the end of the public health emergency.

Non-Nurse Staffing

  • NY ranked worst in the country in respect to average administrator time per resident. Nursing home administrators are responsible for overseeing operations, ensuring compliance with regulatory standards of care, supporting residents’ rights, and maintaining financial accountability.
  • Medical Director. NY nursing homes ranked in the top 20% of states in respect to average medical director time per resident. However, 255 (42.7%) NY nursing homes report an average of zero hours of medical director time for the quarter.
