The Hudson Valley LTC Ombudsman Program is an advocate and resource for individuals who reside in long-term care facilities such as nursing homes, assisted living, adult care facilities, and family type homes. Our program encompasses facilities within Columbia, Dutchess, Greene, Orange, Sullivan, and Ulster counties.
Ombudsmen are resident-centered advocates who empower, educate, and advocate for residents. We also provide support to families so that they have a better understanding of residents’ rights and care standards and are able to make their voices heard on behalf of their residents.
The Program advocates for residents at both the individual and systems levels by:
- Receiving, investigating, and resolving complaints made by or on behalf of residents;
- Promoting the development of resident and family councils; and
- Informing governmental agencies, providers, and the general public about issues and concerns.
Ombudsmen respond to a variety of concerns about long-term care including:
- Quality of care
- Abuse and neglect
- Rights violations
- Lost and stolen belongings
- Dietary concerns
- Discharge, eviction or termination of services
