This page contains facility-level data on Problem Facilities, defined by LTCCC as nursing homes that are Special Focus Facilities (SFFs), SFF Candidates, or assigned a one-star overall rating. Download nationwide data by clicking here. Note: SFFs and SFF Candidates are nursing homes that have a history of serious quality issues or are included in the Centers for Medicare & Medicaid Services (CMS) program to stimulate improvements in quality of care. One-star facilities are assigned the lowest overall rating (one out of five stars) based on health inspections, staffing, and quality measures.
